This free printable checklist, created by Melissa Farkouh, M.Sc.(A), (C), speech-language pathologist, helps the school team document essential information before making a referral: linguistic profile, observation conditions, support already in place, student's response, and family's observations.

The objective: to replace the simple "they are having difficulties" with a much more precise picture of the situation.

The checklist allows you to verify:

• the languages understood and used by the student;
• their exposure to the school's language;
• the contexts in which difficulties appear;
• strategies already attempted in class;
• their response to support;
• family observations;
• useful information to share with the speech-language pathologist.
